Se ha realizado una revisión sistemática siguiendo el protocolo PRISMA para caracterizar la producción científica de la didáctica de la higiene alimentaria en Educación Primaria. Se analizan indicadores bibliométricos y metodologías de investigación utilizadas. La búsqueda se realizó en la base de datos Web of Science. Solo 12 artículos cumplían con todos los criterios de inclusión. Hay escasas publicaciones sobre higiene alimentaria en la etapa, tratándose de estudios cualitativos, autoría conjunta y en su mayoría pertenecen al sector sanitario. Es necesario (...) aumentar las investigaciones especialmente en el ámbito educativo e introducir la higiene alimentaria en la formación del profesorado. (shrink)

Healthcare professionals often face ethical conflicts and challenges related to decision-making that have necessitated consideration of the use of conscientious objection (CO). No current guidelines exist within Spain’s healthcare system regarding acceptable rationales for CO, the appropriate application of CO, or practical means to support healthcare professionals who wish to become conscientious objectors. As such, a procedural framework is needed that not only assures the appropriate use of CO by healthcare professionals but also demonstrates its ethical validity, legislative compliance through (...) protection of moral freedoms and patients’ rights to receive health care. Our proposal consists of prerequisites of eligibility for CO (individual reference, specific clinical context, ethical justification, assurance of non-discrimination, professional consistency, attitude of mutual respect, assurance of patient rights and safety) and a procedural process (notification and preparation, documentation and confidentiality, evaluation of prerequisites, non-abandonment, transparency, allowance for unforeseen objection, compensatory responsibilities, access to guidance and/or consultative advice, and organizational guarantee of professional substitution). We illustrate the real-world utility of the proposed framework through a case discussion in which our guidelines are applied. (shrink)

In March 2021, the Spanish Congress approved the law regulating euthanasia, that regulates both euthanasia and physician-assisted suicide (PAS). In this article, we analyse the Spanish law regulating euthanasia and PAS, comparing it with the rest of the European laws on euthanasia and PAS (Netherlands, Belgium and Luxembourg). Identified strengths of the Spanish law, with respect to other norms, are that it is a law with many safeguards, which broadly recognises professionals’ right to conscientious objection and the specification that it (...) makes on the prior comprehensive care of the patient, including the approach to care dependency. Regarding its shortcomings, the law does not differentiate well between euthanasia and PAS; it barely assigns a role to the healthcare team as a whole (similar to other regulations); it does not clarify the functions of the different professionals involved; it does not detail the specific composition and duration of theevaluation commission; it has not been accompanied by a prior or simultaneous regulation of palliative care; and, lastly, the period of time to implement the law is too short. Many recent discoveries have confirmed the importance of Orphism for ancient Greek religion, philosophy, and literature. However, its nature and role are still very controversial. The key problem of its relationship to Christianity has been discussed by ancient and modern authors from many different viewpoints, albeit too often tainted with apologetic interests and unconscious projections. Introducción: el uso de la alimentación enteral en pacientes con demencia resulta un desafío ético-clínico. El objetivo de este artículo es exponer el proceso de validación de un cuestionario para conocer la opinión de los médicos al respecto y sus resultados preliminares. Metodología: se desarrolló un cuestionario anónimo para conocer la opinión de médicos acerca del uso de la alimentación enteral, dirigido a especialistas de cuidados paliativos, geriatría, medicina interna y neurología, de los ámbitos público y privado. Se presentaban allí (...) tres casos clínicos de pacientes con demencia en diferente grado de evolución. Las preguntas abordaban cuatro campos específicos: motivaciones del inicio o suspensión de la alimentación enteral, adecuación terapéutica, eutanasia y toma de decisiones. Resultados: se obtuvo un índice de respuesta del 64 %. El cuestionario resultó una herramienta con validez y fiabilidad aceptables. Los especialistas en cuidados paliativos y geriatría eran más propensos a adecuar el esfuerzo terapéutico y a integrar a la familia en las decisiones. Los profesionales de medicina interna y neurología tendían a ser más invasivos en los tratamientos. Conclusión: los resultados obtenidos permiten recomendar el cuestionario como metodología para valorar la actitud de los profesionales ante las cuestiones bioéticas más relevantes en la atención a pacientes con demencia. Movies can serve valuable didactic purposes teaching clinical ethics to medical students. However, using film sequences as means to develop critical thinking is not a straightforward task. There is a significant gap in the literature regarding how to analyse the ethical content embedded in these clips systematically, in a way that facilitates the students’ transition from anecdotal reflections to abstract thinking. This article offers a pedagogical proposal to approach the ethical analysis of film sequences in a systematic fashion. This structured (...) stepwise method encourages students to identify the main ethical problem of a selected scene and to reflect on the theoretical principles involved, emphasizing the application of these norms and values in a contextually situated analysis. We believe this method in film studies both reinforces the students’ comprehension of the theoretical framework of an ethical topic, and casts light on its pertinence and limitations under the circumstances of the scene, thus proving a constructive tool to strengthen the bridge between the theoretical teaching of clinical ethics and clinical practice. Propone develar cómo influye el narcotráfico, las formas de resistencia y reexistencia en la escuela del Norte del Valle del Cauca (Colombia). Desde un enfoque cualitativo se forman textos de las historias de vida y relatos de experiencias construidos en diálogo sobre los temas generadores Escuela y narcotráfico, analizados por medio de matrices de interpretación y su triangulación; autores, actores y maestro participante en una perspectiva hermenéutica pluritópica. En ella participan estudiantes, padres y madres de familia, docentes y otros miembros (...) de la comunidad escolar. En sus conclusiones se destaca: la marca colonial en la economía nacional (glocal); el imperativo impuesto por las precarias condiciones económicas; los altos niveles de corrupción que crean un caldo de cultivo para el surgimiento de grupos delincuenciales; la escuela como posibilidad de formación humana que resiste y reexiste al narcotráfico; las contradicciones a las que son sometidos los jóvenes en el enfoque de atención al problema del consumo y la capacidad de algunos miembros de la comunidad para construir un proyecto de vida que les permite reexistir por fuera de las lógicas del narcotráfico como sujetos históricos. We conducted three studies aimed at showing that one-to-one translations between emotion terms might be comparing independent or barely overlapping categories of emotional experience. In Study 1 we found that the speakers' most accessible features of two supposedly equivalent emotions terms (shame and vergüenza) were very different. In Study 2, American and Spanish speakers' typicality ratings of 25 out of 29 constitutive features of “shame” or “vergüenza” were significantly different. In Study 3, these important differences in the content and internal (...) structure of “shame” and “vergüenza” were also reflected in the affective meaning of their corresponding terms. We conclude that “shame” and “vergüenza” do not share the same cluster of central features, and cannot be treated as identical categories of emotional experience. This finding raises serious questions about the one-to-one translation practices not only of most cross-cultural studies on “shame”, but also of other emotion concepts. We suggest an alternative method of translation based on Diachronic Prototype Semantics (Geeraerts, 1997). (shrink)

_Efficacy of Palliative Care in Pain Relieving_ _Eficácia dos cuidados paliativos no alívio do sofrimento_ El alivio del sufrimiento es uno de los principales objetivos médicos. Mediante una escala de medición cuantitativa del sufrimiento se investigó la repercusión de la atención del servicio de cuidados paliativos. El estudio se realizó en pacientes con enfermedades avanzadas atendidos en el principal hospital de referencia de dicha especialidad en Argentina. El 88% de los participantes refirió sufrimiento en la primera consulta. Este porcentaje disminuyó (...) al 44% luego del seguimiento clínico de cuidados paliativos. Este estudio evidencia que la atención médica competente de Cuidados Paliativos logra reducir el sufrimiento, asegurando una calidad de vida adecuada durante la enfermedad, hasta su final natural. De este modo, los Cuidados Paliativos constituyen una respuesta médica y ética válida frente a la eutanasia. (shrink)

The COVID-19 pandemic has generated an imbalance between the clinical needs of the population and the effective availability of advanced life support (ALS) resources. Triage protocols have thus become necessary. Triage decisions in situations of scarce resources were not extraordinary in the pre-COVID-19 era; these protocols abounded in the context of organ transplantation. However, this prior experience was not considered during the COVID-19 outbreak in Spain. Lacking national guidance or public coordination, each hospital has been forced to put forth independent (...) and autonomous triage protocols, most of which were, nonetheless, based on common ethical principles and clinical criteria. However, controversial, non-clinical criteria have also been defended by Spanish scientific societies and public institutions, including setting an age cut-off value for unilaterally withholding ALS, using ‘social utility’ criteria, prioritising healthcare professionals or using ‘first come, first served’ policies. This paper describes the most common triage criteria used in the Spanish context during the COVID-19 epidemic. We will highlight our missed opportunities by comparing these criteria to those used in organ transplantation protocols. The problems posed by subjective, non-clinical criteria will also be discussed. We hope that this critical review might be of use to countries at earlier stages of the epidemic while we learn from our mistakes. (shrink)
